- Your full name.
- The Church or Mission you attend.
- Your location (City, State/Region, Country)
- The URL of your blog
- The subject of your blog (e.g., Movie reviews, Life in the Country, Scientology, Drug Rehabilitation, Human Rights, etc.)
- The email address you want me to use if I need to contact you (if not specified then I'll use the one you email me with)
